Description

Dapagliflozin (Forxiga, Farxiga in the US) is a selective inhibitor of sodium-glucose cotransporter 2 (SGLT2), clinically approved for type 2 diabetes, heart failure and chronic kidney disease. SGLT2 sits in the proximal tubule of the kidney and pulls roughly 90 percent of filtered glucose back into the blood. Dapagliflozin blocks that transporter with more than 1000-fold selectivity over intestinal SGLT1, so reabsorption drops away and glucose ends up in the urine.

The effect is mechanistically simple and that is what makes it interesting: at 10mg daily, studies document roughly 70 grams of glucose excreted renally per day, which corresponds to about 280 kilocalories. This excretion runs independently of insulin, so the hypoglycemia risk in monotherapy is low. Side consequences of the mechanism are an osmotic diuresis with mild volume and blood pressure reduction, plus a rise in hematocrit and hemoglobin.

Pharmacokinetically dapagliflozin is well absorbed orally with an absolute bioavailability of about 78 percent. Peak concentration is reached after roughly 1 to 2 hours in the fasted state, food delays onset but does not meaningfully change total absorption. Terminal half-life is around 12.9 hours after a single 10mg dose, which supports once-daily dosing. Clearance runs mainly through UGT1A9 to the inactive dapagliflozin 3-O-glucuronide rather than primarily through CYP450, which keeps the interaction profile comparatively lean.

In clinical studies dapagliflozin is one of the best documented agents in its class. DECLARE-TIMI 58 (NEJM 2019) with over 17,000 participants missed superiority on the classic MACE endpoint but clearly reduced the composite of cardiovascular death and hospitalization for heart failure. DAPA-HF (NEJM 2019) showed a roughly one quarter reduction of the primary composite endpoint in heart failure with reduced ejection fraction, in participants with and without diabetes. DAPA-CKD (NEJM 2020) and DELIVER (NEJM 2022) extended the picture to kidney disease and preserved ejection fraction.

In the research and fitness context the class is mostly discussed because of the renal calorie loss, sometimes alongside GLP-1 protocols. That is exactly where the central danger sits: euglycemic ketoacidosis occurs at normal or only mildly elevated blood glucose and is favoured by low-carb or ketogenic eating, fasting, alcohol, infections or a steep calorie deficit. Risks & Safety

Dapagliflozin is an approved SGLT2 inhibitor with years of safety data from large endpoint trials. The effect profile is well characterised, but it includes one rare and potentially serious complication in euglycemic ketoacidosis that you need to know about.

Documented in clinical studies:

  • Euglycemic ketoacidosis: the most important rare danger of the class. Ketones rise while blood glucose stays normal or only mildly elevated, so a routine glucose check shows nothing. In DECLARE-TIMI 58 diabetic ketoacidosis occurred in 0.3 percent on dapagliflozin versus 0.1 percent on placebo. Triggers described in the literature are ketogenic or strongly carbohydrate-restricted eating, fasting, alcohol, infections, surgery and dehydration. Nausea, vomiting, abdominal pain, deep breathing or confusion are described as warning signs.
  • Genital fungal infections: the most common documented effect, a direct consequence of glucose in the urine. Women are affected considerably more often than men. In the registration trials the rate sat in the low single-digit percent range depending on the cohort, clearly above placebo.
  • Urinary tract infections: also documented above placebo level, out of the same mechanism.
  • Volume depletion and blood pressure drop: the osmotic diuresis lowers plasma volume and blood pressure. Dizziness, orthostatic complaints and dehydration are documented, amplified by concurrent diuretic use, high sweat rates or low fluid intake.
  • Fournier gangrene: a very rare necrotising soft tissue infection of the genital and perineal area, tracked as a safety signal across the whole SGLT2 class.
  • Initial eGFR dip: a reversible drop in estimated kidney function during the first weeks is regularly documented and transitions into a slower long-term decline than under placebo.
  • Hematocrit increase: hemoglobin and hematocrit rise measurably under SGLT2 inhibition. That is a relevant point when blood values are already elevated.
  • Hypoglycemia in combination: low in monotherapy, clearly more frequent when combined with insulin or sulfonylureas.
