Description

Empagliflozin (Jardiance) is an SGLT2 inhibitor, meaning it blocks the sodium-glucose cotransporter 2 in the proximal tubule of the kidney. That transporter normally pulls almost all filtered glucose back into the blood. When it is blocked, the renal threshold for glucose drops and part of the sugar leaves the body via urine instead of being reabsorbed. The mechanism is therefore entirely insulin-independent - it needs neither a working beta cell nor an insulin response.

The pharmacological difference to dapagliflozin lies in selectivity. In the characterisation work by Grempler et al., empagliflozin had an IC50 of 3.1 nM at human SGLT2 and the widest selectivity window over SGLT1 of all SGLT2 inhibitors tested there: roughly 2500-fold, against about 1200-fold for dapagliflozin and about 250-fold for canagliflozin. SGLT1 sits mainly in the gut, so high SGLT2 selectivity means intestinal glucose uptake stays largely untouched. In practice the clinical effect of both substances is similar - the real difference lies less in the selectivity number than in the body of trial data, which for empagliflozin is unusually broad across diabetes, heart failure and kidney disease.

Pharmacokinetically empagliflozin is absorbed quickly, reaches peak levels after about 1.5 hours and has an oral bioavailability of roughly 78 percent. Terminal half-life is around 12 hours, in individual studies in the range of 10 to 19 hours. The glucosuric effect nevertheless lasts a full 24 hours because it is tied to transporter blockade rather than to plasma level - which is why once-daily dosing is sufficient. A high-fat meal lowers absorption slightly but is not considered clinically relevant, so intake can happen with or without food.

The landmark work is EMPA-REG OUTCOME (Zinman et al., NEJM 2015) with more than 7000 participants with type 2 diabetes plus established cardiovascular disease. Documented was a 14 percent reduction of the combined cardiovascular primary endpoint, driven mainly by 38 percent fewer cardiovascular deaths and 35 percent fewer hospitalisations for heart failure. Notably the effect appeared very early and could not be explained by glucose lowering alone. EMPEROR-Reduced (Packer et al., NEJM 2020) extended that line into a purely cardiological population: 3730 patients with heart failure and reduced ejection fraction, hazard ratio 0.75 for the combined endpoint, and independent of whether diabetes was present. EMPEROR-Preserved widened this to preserved ejection fraction in 2021.

In a research context empagliflozin is interesting because it creates a glucose-dependent loss through urine that comes with a caloric outflow and a mild osmotic diuresis. But exactly that is where the risks come from, particularly in combination with ketogenic or heavily carbohydrate-restricted eating. Anyone wanting to classify the effect profile should not skip the risks section below, because the most relevant complication of this substance class specifically does not announce itself through a conspicuous blood glucose reading.

Risks & Safety

Empagliflozin is an approved SGLT2 inhibitor with a very large trial base and a well characterised effect profile. It shares its risks largely with dapagliflozin and the rest of the class - they follow directly from the fact that sugar is continuously excreted through urine.

Documented in clinical studies:

  • Euglycemic ketoacidosis: the most relevant complication of the class. Rosenstock and Ferrannini described in Diabetes Care that ketoacidosis under SGLT2 inhibitors can occur at normal or only mildly raised blood glucose values, because the sugar drains through the kidney instead of accumulating in the blood. The classic warning sign of high blood glucose is therefore absent. Contributing factors described in the literature include strongly carbohydrate-restricted or ketogenic diets, fasting, alcohol, infections and perioperative situations. The combination of SGLT2 inhibition and low-carb is thus the clearly highest-risk constellation for this substance.
  • Genital infections: the most consistent side effect across all studies. Sugar in urine creates a growth medium; documented above all are genital mycoses, considerably more frequent in women. In EMPA-REG OUTCOME this was the only clearly elevated adverse event category.
  • Urinary tract infections: described in the class, less clearly elevated in the empagliflozin trials than genital infections. Rare severe courses are reported in the literature.
  • Volume depletion and blood pressure drop: the osmotic diuresis causes fluid loss. Documented are blood pressure lowering, hypotension symptoms and dizziness, especially relevant with additional diuretic action, in older users or with already low fluid intake.
  • Initial creatinine rise: an early dip in estimated filtration rate is documented, which realigns over time. In long-term data kidney function was better preserved under empagliflozin than under placebo overall.
  • Effect depends on kidney function: the whole mechanism runs through the kidney. With reduced filtration performance the glucosuric effect decreases accordingly.
  • Hypoglycemia in combination: on its own the hypoglycemia risk is low because the mechanism is insulin-independent. In combination with insulin or sulfonylureas it rises measurably.
